Position: Licensed Vocational Nurse (LVN)

Status: Full-Time/ Part-Time/ Per Diem

Location: Temecula, Murrieta, Lake Elsinore, Wildomar, Corona, Winchester, French Valley

Position Summary : The Licensed Vocational Nurse (LVN) provides direct care under the supervision of a Registered Nurse and in accordance with physician orders, agency policies, and California regulations. The LVN supports patients in the home setting through skilled nursing tasks, monitoring, education, and compassionate care.

Essential Duties and Responsibilities:

• Provides nursing care within LVN scope of practice
• Observe, document, and report patient condition changes.
• Administer medications and treatments as ordered.
• Perform wound care and basic clinical procedures as assigned.
• Assist with disease management education for patients and families.
• Maintain accurate, timely electronic documentation.
• Communicate with RN Case Manager, physicians, and interdisciplinary team.
• Follow infection control and safety standards.
• Participate in on-call or weekend rotation if assigned.

Qualifications:

• Current California LVN license in good standing.
• Current CPR/BLS certification.
• Valid drivers license, reliable transportation, and auto insurance.
• Minimum 1 year nursing experience preferred.
• Home health or community-based experience preferred.
• Strong communication and time-management skills.

Physical Requirements:

• Ability to stand, walk, bend, lift up to 25 pounds, and travel to patient homes throughout service area.

Work Environment:

• Field-based position providing care in patient homes.
