Job Description

Overview:

Starting pay of $36.90, Pay will commensurate with experience

Schedule: Typically 8am-5pm, Day shift, traveling around Franklin County Area

Valid Ohio driver's license and proof of auto insurance is required by hospital policy. Must pass a motor vehicle background inspection, insurance eligibility, driving qualifications and training set forth by Nationwide Children's Hospital and must maintain qualification of insurance guidelines.

Job Description Summary:

Supports new parents in providing newborn care, offers physical assessments, addresses questions, identifies needs, and helps to identify and facilitate referrals to community services or resources. Provides home visits in the determined catchment area and works in a multidisciplinary environment to schedule, visit, and support families. Adheres to the FC model, and to policies, procedures, guidelines and standards. Practices professional nursing and promotes patient advocacy in accordance with the ANA Code of Ethics for Nurses and maintains a current Ohio Nursing License.

Job Description:

Essential Functions:
  • Follows protocols, records observations and activities, and documents information for family assessments and program evaluation. Records information in real time via in-home use of computerized record systems and in compliance with state guidelines. Visits target area hospitals as needed to schedule home visits for new mothers.
  • Provides a health and physical assessment of mother and baby in the home to determine well-being. Assesses family in 12 domains of physical and psychosocial well-being, including depression, substance abuse, and domestic violence.
  • Establishes a trusting relationship with families during in-home visits and engages the family to establish a support plan with goals, objectives, and activities.
  • Collaborates to ensure appropriate referrals and connections to community resources, acting as a liaison and advocate. Assists parents in making and attending appointments as needed.
  • Applies working knowledge of parent-child interaction, child maternal health, child development, and child abuse or neglect to help families improve parenting skills, increase parenting supports, reduce stress, and address needs of parents.
  • Maintains eight open visit slots. Conducts and documents at least six home visits per week. Responds to client needs through flexible scheduling of home visits.
  • Attends weekly team meetings, clinical supervision, staff meetings, and other meetings. Supports and encourages the development of staff and programs. Identifies gaps in critical community-wide resources with the goal of working toward increasing needed services locally.
Education Requirement:
  • Associates degree in Nursing, required.
  • BSN, preferred.
Licensure Requirement:

Active Ohio RN license, required.

Certifications:
  • BLS, required.
  • Certified Lactation Educator or Consultant, required.
Skills:
  • Proficient and comfortable using laptops as part of in-home assessment and intervention.
  • Ability to work on a team of clinical and non-clinical professionals.
  • Effective time-management skills, organization, and multitasking skills.
  • Exercises clinical skills in appraising situations and making decisions.
Experience:
  • Clinical experience related to domestic violence, substance abuse and/or depression, required.
  • One year of experience in child/maternal health, social work, or another related field, required.
  • Experience Working with diverse populations, required.
